Ultrasonic liposuction, UAL, is a technique to enhance the performance of liposculpture. A
generator is used to produce ultrasonic waves which are transmitted to the suction cannula.
When the cannula comes in contact with the fat cell membranes, they are disrupted
through the process of cavitation. The emulsified fat cells release triglycerides
and free fatty acids which are removed via suction.
Ultrasonic liposuction can be performed with other procedures including a Facelift, Necklift, and Tummy Tuck. UAL is almost always done in conjunction with traditional Liposuction with the tumescent technique. The perceived benefit over traditional liposuction is the cannula can pass through the tissue more gently because the ultrasonic waves are doing the work and less pressure can be used for extraction. Thus, the procedure is less physically taxing on the surgeon. UAL is especially beneficial for use on the back where there is a more extensive fibrous network which impedes the ability to extract the fat. It also appears to be beneficial for the removal of excess breast tissue in males (Gynecomastia) which too possesses an extensive fibrous network. UAL may produce better skin contraction either through its ability to selectively emulsify the fat or through an unknown direct effect on the skin. However despite its theoretical advantages, UAL has not replaced traditional liposuction techniques. The major disadvantages include larger cannulas requiring larger incisions for insertion, potential for full thickness skin burns due to the heat generated from the special cannulas, increased cost, operating times which are nearly double the traditional method and it cannot be used for superficial liposculture due to the heat generated by the cannula. New cannulas are currently being used which have a sheath for skin protection or a cooling mechanism which will lessen the risk for burning. Finally, UAL is contraindicated in patients with cardiac pacemakers. In conclusion, no scientific publication has shown conclusively that the final aesthetic outcome of UAL is better then traditional techniques of liposuction utilizing the tumescent technique. |
